Overview

During this interactive session expert faculty will guide attendees into key elements and strategies in communicating difficult news to patients and their families, including standardized approaches in initiating these conversations and how to respond to emotion. 

Learning Objectives.

  1. Describe key factors in communicating difficult news 
  2. Identify skills to improve your communication with patients and families 
  3. Recognize that emotional cues need to be satisfied PRIOR to knowledge-based communication
  4. Employ “SPIKES” as a talking map for communicating bad news 
  5. Practice using a “warning shot” and a “headline/summary” 
  6. Apply “NURSE” skills to respond to emotion

Target Audience

This activity is intended for oncology nurse practitioners, physician assistants, nurses, nurse navigators, medical assistants, and other healthcare providers involved in the care of cancer patients.

Dee Ann Omatsu, MS, APRN-Rx, PNP-BC, CPON 

For the past year, Dee Ann Omatsu has been the clinical oncology educator for Hawai‘i Pacific Health, covering both adult and pediatric oncology education and clinical program development at Kapiolani Medical Center, Pali Momi Medical Center, Straub Medical Center and Wilcox Medical Center. She is responsible for ongoing education for nurses and oncology staff as well as maintaining clinical competency. Dee Ann also assists with clinical program development within the oncology service line and is the liaison for staff nurses and advanced practice nurses in facilitating research at our facility. 


Previously Dee Ann was the pediatric oncology nurse practitioner at Kapi’olani Medical Center for Women’s and Children. As a member of the pediatric oncology team her work scope consisted of a variety of duties. She was responsible for direct care of children and adolescents who have or have had cancer. Dee Ann coordinated the first teen and adolescent cancer support group at Kapi’olani Medical Center, founded in 2010. She also coordinated the Long Term Survivorship program at Kapi’olani Medical Center and started the first AYA Survivorship Transition Program.  As a second occupation, Dee Ann is a clinical faculty member at Hawaii Pacific University as a part-time nursing clinical instructor for the bachelors in science nursing program. 
 

Dee Ann’s role as the pediatric oncology nurse practitioner at Kapi’olani Medical Center has provided the opportunity to assist with the diagnosis, treatment and supportive care of children and adolescents with cancer in all phases of their disease; initial care, active treatment, terminal care or long-term follow up care. Her current position as the clinical oncology educator allows the opportunity for professional growth and development beyond just herself at the bedside. She now has multiple areas to influence all the clinical hands that touch oncology patients improve the care that they receive. 

 

Rachel Thelen, MSN, FNP-C, AOCNP

Rachel Thelen is an Oncology Certified Nurse Practitioner at the Maui Memorial Medical Center
Outpatient Clinic. She received her Bachelor of Science in Nursing from UNC Wilmington in 2009 and Master of Science in Nursing from Western Carolina University in 2016. She has over fourteen years of hematology/oncology experience. She began her career as an inpatient oncology nurse, followed by outpatient infusion and now, as an advanced oncology practitioner since 2016. She relocated from the east coast to Maui in 2020 with her husband, John, and daughter, Ruby.

 

Justin Baker, MD (Speaker)                                                                                                        

As a Pediatric Oncologist, Palliative Care Physician, and Phase I and End– of– Life Care Clinical Investigator, Dr. Justin Baker is intimately aware of the distress experienced by children with advanced cancer and the ethical and end– of– life/bereavement issues surrounding their disease progression. He currently serves as the Chief of the Division of Quality of Life and Pediatric Palliative Care here at Stanford, as well as the Director of the Quality of Life for All (QoLA) Program. Additionally, Dr. Baker serves as the Associate Chief Quality Officer for Patient Experience and Holistic Care. In his past career at St Jude, he served as the Director of our large Pediatric Hematology/Oncology Fellowship Program for more than a decade. 

Dr. Baker's research interests include ethical considerations surrounding enrollment in Phase I clinical trials, AYA palliative oncology care, end– of– life decision making, grief and bereavement, integrating palliative care into the ongoing care of children with cancer as well as patient– reported outcomes and pain and symptom control in the context of pediatric oncology care. He has received significant extramural funding for his research, and Dr. Baker has participated in dozens of studies related to pediatric palliative care. He has authored ~250 academic works on palliative care subjects within the context of pediatric oncology. In sum, Dr. Baker is a recognized global expert and leader in the field of Pediatric Palliative Care.
